I am not too sure but I don't think you'll likely find a contract as you seek because if that were the case, outside of professional makeup work for a major studio or company, that kind of contract would favor the company and not the artist. Why you're finding mostly contracts for artists is because most of the time they are independent contractors (even if they belong to union often they do work where and when they can get the work - though they're not supposed to) and they create their own contracts to protect themselves because historically there are unscrupulous studios, production companies or others, bent on cheating them out of what they're due.
I've seen it too many times where artists are treated very badly and get cheated out of money that's due to them, their kit fees for their supplies and such, which often get abused or stolen, etc., and so most of them have created contracts which are legally binding to protect themselves and their interests.
If you intend to hire a professional makeup artist (and I URGE you to be VERY VERY CAREFUL who you hire) then simply create a contract that details what is expected, how the work is to be delivered, when, where, etc., etc....you get the drift - and be sure it's agreeable with the artist. HOWEVER, MOST artist's contracts do define what you and he or she have agreed upon in discussions ahead of time. And while they do favor the artist in those cases, also they're usually structured to protect the company for whom they are working also. It's a reciprocally protective contract for both parties in most cases....if written properly. But if you want to create your own, they have to be afforded the rights as well which will protect their investment of time, materials and so forth just like you would want. Just do what is fair and then you both have opportunity to discuss the terms ahead of signing anything.
And again, they do their own contracts simply because of the pattern and history in many cases, of being used and abused by companies who hire them and then cheat them out of what is rightly their terms. Tough one to find - I'd say it's going to be. And further, as I said, be absolutely sure whom you're hiring and make sure they are highly qualified (MOST ARE NOT). You wind up with a lot of makeup artists who are under-trained, over-confident, and amateurish at best. So be very very careful, especially since you want this educational experience to be the best it can be to teach this highly specialized craft properly and fully. Get someone who really knows what they're talking about and has the credentials to prove they can accomplish what you need done for training or for a specific project or series of projects.
I cannot begin to tell you how many times I've heard people say they're makeup artists, or actors who "do their own makeup", only to find the end results are amateurish and ridiculous at best. But then I've been doing it for almost 50 years, have studied it that long (it's a life-long pursuit), have credentials in special makeup effects from an Academy Award winning "godfather of modern special makeup effects", and have been involved in theater mostly for that entire time. And PLEASE I am NOT boasting - it's simply the facts. Yes, I have MY own contracts but they've been developed over a period of the last 30 years of doing projects for people and learning my lessons well. SADLY they just don't teach actors or makeup artists in college as they should. That kind of work is almost left to the individual to learn on their own. Some do learn, most do not understand the intricacies of good theater makeup work - much less the basics...because colleges, schools, etc., simply don't teach it enough. They all say "go buy Corson's book" and then don't do much to teach them from that book. And there are tons of books out there - most of which are garbage when it comes to theater makeup work...except Corson's. But it takes someone with artistic talents almost to really be able to develop skills that exceed the application of one color of makeup (wrong) and a bit of other work to enhance an actors work. And when it comes to doing anything "special" that might be required, there are few who can do it properly.
I've said it before and I'll say it again SOME DAY I am going to do a book or series of DVD's for BASIC and ADVANCED theater makeup work. BUT I THINK I'll have to retire before I have the time - that is just 3 more years. Then hopefully I can pass along some of what I've been blessed enough to learn over the last 50 years of doing this.
Good luck. But I think you may find little or no information on contracts as you define. I'd say go with the artist's contract but read it well and fully before committing as well as discussing it in a meeting to lay out the parameters and needs. Leave NOTHING to chance.
