Is there a standard format for script writing that is "supposed" to be
followed? I've seen various styles: character names capitalized and not
capitalized, character names indented above the lines and centered above
the lines, stage directions in italics or not, etc. etc.

There are very clear printed formats for plays, musicals and screenplays.  Pick up some word processing software like Final Draft and you're in business
