I will be directing Gore Vidal's The Best Man with auditions starting next week.  I'm not exactly in love with our theatre's audition sheets and was wondering if any of you had examples of what your theatres use.  After over 20 years of comedies and musicals this will be my directorial debut into the world of drama so wish me luck.

This is not an audition form, but it may help!
It helped the production when I directed A Chourus Line.
 

OBLIGATION AGREEMENT

This is an obligation and agreement of conduct by the undersigned, in accord with Theatre Policy, as it is deemed from to time.

By the acceptance of particaption, that all those choosen, as defined by the date of signing hereto, in the production &/or event.

Shall become a recognised as a financial member of the theatre, in the prescibed manner.

I further agree to being punctual in attendance to all scheduled meeting, rehearsals, performances or other events that are as agreed &/or deemed normal course of practice for the type of production/event season.

I realise it is my responsibility, as the participant, to advise immediatly of any possible or unaviodable delay in attendance &/or by giving prior adaquate notice of any other occasion that may arise.

In the manner prescribed, at the commencement of the production/event process.

Except in emergency situations that are deemed an acceptable occurance.

That this & any other unsuitable conduct of the participant may result in suspention or exclution. Pursuant to conduct of that deemed unbecoming a member &/or participant.

I understand the reasons for this policy & the obligations applied by this agreemant.

In accord with the Incorporation Act Constitution & it's subsiquent policy, orginisational rules & regulations of the Theatre Inc.

I ask those auditioning to bring a resume (and they're kids), but have some blank forms available for newbies to complete.
 
In a nutshell, I need their name, address, email, telephone/cel number, stats (hair/eye color, height, weight, vocal range), past experience (role, production, theatre), training (workshops, school), and special skills (sports, dialect, combat, voice, etc.). 
 
I then make notes directly on their resumes during the audition RE:  in what part they're interested, if they'd play a role of the opposite gender (we are very girl heavy), if they would be interested in working backstage if not cast, etc. 
 
Having a camera on-hand (especially if they've forgotten a picture) is also important for me for when I get down to the actual casting ... I can lay the pix beside one another to group families, couples, etc.

Hey Patrick,
For Pippin, the director wanted poloroids of everyone that showed up to audition.  He paperclipped them to the audition form that the actors filled out.  We also use different directors for different shows.  We start with a form and if the director doesn't like it, they will create their own.  But, as the theatre organization, we act as the producer. (I know some theatres have one person that is the producer for the play).  But, as the theatre organization, we want to capture information from people that show up for auditions.  Mainly: name, address, phone number and e-mail so we can add those people to all of our lists.  So, we always proof to make sure that information is on whatever form is used.
 
We have actors fill out an audition form at the auditions.  The form includes small calendars with dates filled in, up to the last performance date.  On the calendar, we already have filled in the tech week times and the performance times and dates.  Then, we ask people to list any conflicts they have between now and the last performance.
 
The top half of our form has the basic info to fill out: name address, cell ph., work ph., home ph., e-mail address.
 
The middle is the three calendars side by side.
 
The bottom half of the form has: Part(s) interested in:_______________
___ I will accept any part offered
___ I only want to be considered for the part(s) lilsted
 
I prefer Sunday rehearsal at:__1-4  __4-7  __7-10  __makes no difference
Employer/school_______________ Occupation___________________
Job title/school grade or major__________________________
Schools attended/degrees earned_______________________
__________________________________________________
spouse/parents__________________ children_________________
List any theatre, dance, music, instrument or vocal training

Write any additional talents like juggling, gymnastics, play any instruments, or anything else that is theatre or arts related.
 
Then, the only other thing I would add is that many people abreviate or do not list a lot of information on the audition form.  So, those people that are cast, we have them fill out a "bio sheet" at the first rehearsal and read through.  We want the playbill to appear professional and reserve the right to edit the bios.  We tell people to include education, degrees earned, current job and occupation (or school) and then list all of the plays, characters played, theatre name, etc.  We tell them to be complete  (we want the bios to be uniform) and not to write crap like "I'd like to thank my cat, Fluffy".  That stuff won't end up in the bio in the playbill anyway.

You could also add a question something like "If not cast, would you be interested in working on the play in any other capacity?" You can get new volunteers that way,

OH, yes, yes, yes.  I forgot about that.  At the bottom of our audition form there is a list of the different areas to volunteer from back stage to front of house.   for the longest time, our problem is that we were not tracking that.  People would check an area they were interested in (or multiple checks) and then we were not following up and tracking it beyond that particular play.  We finally started to capture that and enter it into a volunteer data base.  Yes, good catch Jayzehr, forgot about that.

I pretty much have it down. I only had to call "line" twice last night. Still working on those places where a word or two always seems to pop out wrong and a couple of spots where I always seem to forget what's next--you know the drill:)
